Special AVIT form (Minister / Minister of Music

The objective of this form (Next two screen shots.. overlapping due to screenshot) is to provide a quick method for the ministerial staff (Presently the Minister and Minister of Music) to communicate needs for upcoming dates to the AVIT Action Group. Check boxes and information entry areas are provided for additional information that the form doesn’t cover. These areas expand as required.

This dropdown box allows for quick selection of the date.

The event date is the same as this one.

This portion of the form (shown on this page and the next) allows the name(s) to be entered and the adjoining dropdown boxes allows selection of the type of microphone required and the intended use of that microphone.

This area has been expanded to show the available choices. To save page space this box is a “Scroll down” type. As instruments are chosen, they are listed in the box below it (also a “Scroll Down”) in order to present a list of the final choices.
