In regards to the communications problem, one solution would be to amend the document. I'll be giving Brian H a proposal in that regard. Earl, for me, it would be easier if the AVIT form was directly attached to the booking form. It's not often easy finding the document and to be honest, I dislike bothering busy people for one form...
